The Bureau of Child Care Services is launching a new application form and additional supporting documents that is comprehensible and family friendly. As the program anticipates an expansion of direct services, new features and coherent information have been included to better serve the needs of the families in Guam.

Here is a list of the updated forms to be launched on February 1, 2023:
- BCCS - Application Form (for families)
- BCCS - Application Form (for foster families)
- BCCS - Employee Earnings Statement Form
- BCCS - Verification of Employment Form
- BCCS - Self Employment Income Form
- BCCS - Medical Consent Form
New Key Features of Application Forms
Application forms have been updated to reduce the burden of errors for clients. All applications and forms have a refreshed layout and design that is user-friendly. The application form is broken down by sections for easier navigation and additional spaces have been added to retrieve all required client information. Breakdown of necessary documentation is also provided to help clients better understand the requirements for eligibility determination.
Measured and Optimized Forms
Information that is gathered in all forms are now easy to understand for the consumers and has improved efficiency for internal processing. Our Bureau seeks to require less forms and documentation from clients to reduce the turnaround time of application processing.
Expansion of Our Services
The expansion of our services is committed to assisting the overall sustainability and ongoing engagement with families. The updated application form for families and foster families draws special attention to some new features including the voluntary referral to other community services, opting into email or SMS notifications, and an additional resource directory for other DPHSS services.
